Planning in advance helps personalise that care and ensure care teams understand how to support. This workshop introduces ACP as conversations over time to help people, patients and families plan.

This introductory session aims to give an update on TEP Version 12. It will help you to understand your responsibilities around managing the care of a patient at end of life who has a TEP form in place. Highlight when it would be appropriate to request a TEP and the importance of the supporting conversations. Please note this is awareness training only and will not enable you to write TEPs.
