Learning Objectives:

  • To be able to know how to access the 'Planning for the future hub' and share the site with patients and families.

  • To be able to identify and demonstrate the communications skills that facilitate a sensitive Advance Care Planning Brief Intervention (ACP BI) conversation.

  • To demonstrate the use of the ACP BI in triads.

Prior knowledge and training on Advance Care Planning is essential.
